OFFICE BASED SURGERY
The continuous evolution of the technology associated with the comfort and convenience of the medical office, have produced a marked change in the way many surgical procedures are carried out. Many surgeries that would only be possible to perform in hospitals and more elaborate surgical centers are now carried out in offices. This change in the location of the service provides important savings in the provision of health care, while increasing the convenience and comfort of patients and allows the physician an extreme ease in scheduling surgeries. It requires, on the other hand, strict security measures and established quality standards.
Office-based surgery is any surgery performed by an accredited surgeon in the environment of a properly equipped medical office and requiring some degree of anesthesia.
The American Society of Anesthesiologists (ASA) defines the various types of anesthesia,
Level 1 - Minimal sedation or (Ansiolise)
It is a drug-induced state in which patients stay awake and respond to verbal stimuli. Although cognitive functions and motor coordination may be affected, ventilatory and cardiovascular functions are unchanged.
Level 2 - Moderate Sedation (Conscious Sedation)
A state of depression of drug-induced consciousness in which the patient responds to verbal stimuli with or without tactile stimulation. No maneuvers are required to keep airways patent and there is spontaneous ventilation.
Level 3 - Deep Sedation (Analgesia)
A state of depression of drug-induced consciousness, in which the patient does not wake up easily but responds coherently with painful stimulation. The ability to breathe spontaneously is compromised. Assisted ventilation may be required. Cardiovascular function remains unchanged.
Level 4 - General anesthesia
A state of medication-induced depression of consciousness, in which the patient does not wake up, even with painful stimulation. There is no spontaneous ventilation and cardiovascular function is impaired.
In our clinic, interventions are performed using only the anesthesia levels 1 and 2 above.
The recovery is always supervision by a doctor or a nurse until discharge of the patient.
